Lessons from Spain
- In a decentralized system, published and evaluated regional plans were followed by service growth, and a sustained regional program could later widen to advanced chronic illness.227,225,226
- Capitation that ignores rurality, plus unpredictable public payment, pushed services toward charity. Stable, need-weighted funding protects equity.228
- Controls on one opioid formulation cut its use sharply, but substitution was incomplete. Regulators should track what patients are switched to.223
- City-led compassionate community programs can widen the circle of people caring for someone near the end of life, though current evaluations come from the programs' own leaders.514,515
- When prescribing rules for strong opioids are simplified, tracking dispensing alongside opioid-related deaths gives an early signal for both access and harm.722,222

Catalonia's program began as a WHO demonstration project with the regional health department and built home, hospital, inpatient, outpatient and psychosocial teams. In 2011 it widened to advanced chronic conditions with a tool to identify people in need. Across Spain, regional plans were followed by growth in services.225,226,227

Spain was not listed among European countries with an official palliative medicine specialization in 2014. Palliative nursing is not a recognized specialization, and a minority of medical schools require palliative coursework.229,230,231

In 2012 Spain removed the extra authorization, a duplicate prescription, needed to prescribe strong opioids. In Catalonia, monthly strong opioid use fell 3.15% afterward while the study's measure of opioid misuse rose. National opioid-related deaths ranged from 14 to 23 per 1,000,000 inhabitants from 2000 to 2019 and tracked prescribing, which researchers propose as a surveillance signal.722,222

Among cancer deaths in two Madrid districts in 2005, 6% occurred in an inpatient hospice, compared with 74% in hospital, 17% at home and 3% in nursing homes. Hospital death was less common in the district with a palliative home care team (61% versus 77%). The data cover two areas of about 200,000 people each and one year.368

- Family caregiving
In Seville primary care, family caregivers of palliative patients often keep a pact of silence about the terminal diagnosis, sometimes using deceit to conceal it while suspecting the patient already knows. The study links this practice to harder coping with death and mourning.516
Subnational evidence- Volunteers and compassionate communities
In Sevilla, the Sevilla Contigo compassionate city program used community promoters to build care networks around people with advanced illness. Among 83 participants, the average number of people involved in care rose from 3.6 to 6.1; the evaluation was done by the foundation that runs the program.514,529
